Prescription Refill Requests
- Please contact your pharmacy first in order to receive your medication(s) in a timely manner.
- Contact your pharmacy 2 to 3 days before your prescriptions run out.
- When contacting your pharmacy for a refill, you will need the prescription number that is printed on the bottle label.
- Bring your prescription information and/or bottle(s) with you for each office visit.
- Please check your prescription refill needs before your appointments.
Hospitalization
Sometimes a patient's condition may require hospitalization. Our physicians admit patients to OSF St. Mary Medical Center in Galesburg, OSF Holy Family Medical Center in Monmouth, OSF Saint Francis Medical Center in Peoria, or any other hospital of your choosing.
